Instructions: Answer all questions.
Question 1 (10 pts)
With reference to any hypothetical Process Industry of your choice; NAME the industry and outline:
a)
The process safety measures or rules that can be put in place to ensure a safe working environment.
b)
The loss prevention techniques that can be put in place to avoid all forms of losses.
Question 2 (15 pts)
Define the following terms as applied in the Chemical Process safety
a) Safety b) Hazard c) Risk d) Hazard rate e) Protective system f) Demand rate
g) Fractional Dead Time
Question 3 (5 pts)
Define Fatal Accident Frequency Rate (FAFR) in terms of
a)
In terms of exposed hours at work
b)
In terms of working lifetime
Question 4 (5 pts)
Using ICI standards (worker works 5 days/week, 8 hours/day, 40 years/lifetime and an overtime of 16800
hours/lifetime), show that for FAFR, number of deaths/10
8
exposed hours is equivalent to number of
deaths/1000 lifetimes.
Question 5 (10 pts)
a)
In a tabular form, and with respect to Industrial Law, contrast the “Tort of Negligence” and the “Tort
of Strict liability”.
b)
How are these two laws related?
Question 6 (10 pts)
In a case between X (Plaintiff) and Y (defendant), without any negligence involved, water overflew from
Y’s home and damaged a farm and crops in X’s (neighbor’s) land.
a)
As a process scientist, under which law should this case be treated?
c)
Which is the rule that should guide the judgment of such a case? Sate this rule.
d)
Say why the defendant was not held liable.
Question 7 (5 pts)
The cost of purchasing a Precision Chemical Balance by a Process Industry was 750 000 FCFA in 1979.
Estimate the present day cost (2015) if the cost index for 1979 was 175 and 250 for 2015.
Question 8 (10 pts)
Some projects are simple and others are complex. Simple projects are easily evaluated by simply comparing the
capital and the operating cost. For complex projects, more sophisticated evaluation techniques have to be used
to evaluate the project. The most frequently used of these techniques is the “Cash Flow Diagram”. Draw the
Cash Flow Diagram for a project and explain the various regions.
